If you are going on a trip to South America for an extended period of time, then it's likely you'll be moving on through various locations and won't need to book all of your accommodation upfront. However, what we would recommend is booking your first few nights for whichever city you fly into. This way you can get into a taxi and have a known address of a hotel or hostel to head for, rather than trawling round one of South America's major cities not knowing where you are.
If your trip coincides with any major festivals such as Carnival, Easter, Christmas or New Year, it would certainly be necessary to book in advance, especially in the more popular locations such as Rio de Janeiro, Salvador, Cusco etc..
